Please send us the following information in the order notes or by emailing members@hepworthwakefield.org: the name and contact details of the recipient, and indicate postage to the recipient or yourself.

For Joint Memberships: please notify us of both Members' names and addresses.

For Young Members* memberships: please include the birth date of the Member.
